    Thoracic outlet syndrome: efficiency of surgery

    Aim: In this study, we aimed to evaluate the effectiveness of surgical treatment in patients diagnosed with thoracic outlet syndrome. Patients and Methods: This study was conducted by analyzing the age, sex, occupational distribution, anamnesis and physical examination findings, symptoms, preoperative examinations, operation findings, postoperative complications, postoperative hospital stay and operation results of thirty seven patients who were diagnosed with Thoracic Outlet Syndrome, and were operated on between 1991 and 2004. Results: Most of the patients were in the 15-35 age group (72.97%) and female (91.9%). Most of these cases were housewives (51.4%). The most common symptoms were pain (94.6%) and numbness (78.4%). The most commonly used test in physical examination was the Adson test, which provided an 86% positive rate. The most common pathology encountered in radiological tests was cervical rib anomaly (37.8%). It was found that EMG of upper extremity supported TOS with a rate of 48.6% and arterial doppler of upper extremity supported TOS with a rate of 13,5%. We applied first rib and cervical rib resection scalenectomy and cutting of fibromuscular bands as a surgery by transaxillary approach. A 78.4% complete recovery was observed in the early period after surgery and there was no mortality in any of the cases. Conclusion: In this study, the results were satisfactory in TOS patients on suitable cases and with good surgical technique. Surgical success rate can reach up to 97%.

    Investigation of the Endothelial Response of the Super Elastic Braided Stent: An Experimental Evaluation

    Aim: This study aimed to experimentally investigate the vascular biocompatibility of the new super elastic braided stents and ultrasonographically follow up the mid-term outcomes on the blood flow of stent-implanted vessels. Method: After designation of the study steps, stents were implanted into the iliac arteries of two ewes. The preoperative and postoperative blood flow and local findings were noted. The ewes were inspected periodically for abnormal body changes, and the stent-implanted vessels were checked with Doppler ultrasonography. The subjects were sacrificed at six months, and the iliac arteries were examined histopathologically. Results: During follow up an occlusive pattern was not observed in the stentimplanted vessels. The histopathological analysis revealed that both two stent area had no significant neointimal hyperplasia, despite sufficient re-endothelisation at the stent surface. Occlusive or stenotic patterns were not detected in both macroscopic and microscopic findings. Conclusion: According to the mid-term findings, our results show that the new super elastic bare stents have good vascular biocompatibility with high patency rates.

    Evaluation of exposure and awareness of radiation in healthcare professionals exposed to ionizing radiation

    Aim: To evaluate radiation exposure and awareness of radiation in healthcare professionals exposed to ionizing radiation. Material and Methods: The study was carried out at the Alanya Alaaddin Keykubat University Training and Research Hospital and the Süleyman Demirel University Hospital. Physicians, nurses, paramedics, anesthesia technicians, caregivers and cleaning staff exposed to radiation, participated in the study. An inquiry form was prepared to carry out the study, in which the demographic characteristics of the healthcare professionals, the characteristics related to the radiation they were exposed to, the effects that may occur due to this exposure were asked; the relationship between these was statistically analyzed according to the answers. Results: One hundred twenty-nine healthcare professionals, 81 men and 48 women, participated in the study. Most of the healthcare professionals exposed to radiation have been working for more than 10 years and at a distance of less than 3 meters to the radiation source. Thirty-two of the participants (24.8%) did not use any personal protective equipment. On the day of exposure to radiation, 91 persons (70.54%) were experiencing symptoms such as weakness, fatigue, and headache, unlike other days. It was determined that personal protective equipment use did not correlate to education levels and radiation training background. Conclusion: Most of the healthcare professionals exposed to radiation have not previously received any training on radiation and do not use personal protective equipment regularly. This situation causes these persons to complain of weakness, tiredness or headaches at the end of the day.

    Increased Cesarean-Section Birth Rates and Affecting Related Factors

    Aim: The present study aims to evaluate the opinions of physicians on increased C-section delivery rates and their coping ways working at different institutions. Method: This descriptive, cross-sectional study included a total of 200 obstetricians and gynecologists with an experience of at least 20 years chosen through simple random sampling among registered in the Republic of Turkey, Ministry of Health da-tabase. In the questionnaire, their opinions about test exams and their opinions about solutions to decrease C-section rates were investigated.Results: The majority of the participants (89.0%) responded “Strongly Agree” to the following item: “Factors other than medical causes may affect the decision for C-sec-tion”. Similarly, the majority of the physicians responded “Strongly Agree” to the item on the increase in C-section rates along with malpractices and social pressure put by the patient and her relatives(89.0% and 89.5%, respectively). For the majority of the participants (84.0% and 85.0%, respectively), the main suggestions to overcome the increased C-section delivery rates was to lower; social pressure put against the physicians by the patient and her relatives, and removal of malpractice penalties re-latedfor possible adverse outcomes during normal delivery. In this study, the majority of the physicians responded “Strongly Disagree” to the items related to the imposing penalties and granting bonuses (56.0% and 56.5%, respectively). Similarly, 81.0% of the physicians responded “Strongly Disagree” to the item stating that healthcare planners correctly interfere with the main cause of increased C-section rates.Conclusions: The most important reasons for the high cesarean rates are seen as the fear of malpractice and social pressure of physicians working in both public and private sectors.

    Osteoporosis (OP); It is a metabolic bone disease characterized by decreased bone mineral density (BMD) and bone strength, increased bone fragility and fracture risk, and deterioration in the microarchitecture of bone tissue. The most important cause of morbidity and mortality in OP are fragility fractures such as osteoporotic hip fractures. Prevention and treatment of osteoporosis can prevent hip fractures and comorbidities. In this paper, the available information about Osteoporosis and Osteoporotic fractures is briefly reviewed.

    What is gray literature and how to search for it

    The term gray literature refers to printed and/or electronic research that has either been published or remains unpublished, in non-commercial form. Many systematic reviews and meta-analysis do not include gray literature sources and studies published in languages other than English. Their inclusion increases accuracy and can reduce systematic errors. It is very important to include gray literature in the screening of sources while conducting scientific studies, in order to obtain evidence-based results. The Acta Medica Alanya journal includes gray literature, in particular in its compilation and editorial writings, and supports citing articles from our country, including those written in Turkish, in its research articles.

    The Effects of Tumor Localization on Small Cell Lung Cancer and its Association With Prognosis

    Introduction: Lung cancer is classified as small-cell lung cancer (SCLC) and nonsmall cell lung cancer (NSCLC), both as pathological subtypes. SCLC is associated with a significantly short life expectancy, and it constitutes 10-15% of all lung cancers. Previous studies showed that lung cancer is mostly dominated by the upper lobe and is more common in the right lung than in the left. The principle aim of this study is to analyze the localization of the tumor in the right and left lung in aggressive and malignant SCLC patients by comparing it with determinants such as anatomical features, demographic features, laboratory features, including the association with peripheral-central localizations, especially overall survival. Methods: There were four hundred forty-six lung cancer patients diagnosed in a chest diseases clinic in a tertiary training and research hospital between 2014-03-31 and 2020-03-31. Of these, twenty percent (n=90) were diagnosed as SCLC. Among ninety patients, six were excluded from the study due to incomplete medical SCLC records, and finally, eighty-four patients with SCLC were included in the study. Results: We classified eighty-four patients into two groups as right and left lung localized SCLC and analyzed all the data. We found that the left lung tumor group had the more extensive-stage disease and had significantly high CRP levels (p=0.027, p=0.045, respectively). When we analyzed the data, such as demographic characteristics, diagnostic methods, overall survival, treatment characteristics, stage characteristics, anatomical features of the right and left tumor groups, we found that there were no significant differences. We used univariate and then multivariate analysis for survival. We found that being sixty-five years old and over (p=0.014), high CRP levels (p=0.016), having centrally localized tumors (p=0.01), having poor performance status (p<0.0001), and having no treatment for primary cancer (p=0.001) were associated with worse survival. Conclusion: Primary treatment of SCLC patients should start promptly. We found that the central location of the tumor as anatomical localization may be associated with worse survival and that the left lung tumor group had the more extensive-stage disease, with significantly high CRP levels. Being sixty-five years old and over, high CRP levels, having poor performance status and having no treatment for primary cancer, were all significantly associated with worse survival.

    The Relationship Between Blood Neutrophil to Lymphocyte Ratio and Tumor Size, Tumor Number, Macrovascular Invasion in Patients with Hepatocellular Carcinoma

    Aim: We aimed to show the relationship between blood neutrophil to lymphocyteratio that is systemic inflammatory marker and tumor s ize, tumor number, macrovascular invasion at the time of diagnosis in patients with hepatocellular carcinoma.Methods: A total 48 patients diagnosed with hepatocellular carcinoma were includedin our study. The patients were divided into two groups according to themedian neutrophil to lymphocyte ratio.Results: A total of 48 patients (11 female, 37 male) were included in our study.The mean age of the patients were 67.18±9.51 years-old. The median neutrophil tolym-phocyte ratio was 3.17. There were significant association between neutrophil tolym-phocyte ratio > 3.17 and macrovascular invasion, the tumor number> 3, thetumor size> 5 cm, Child-Turcot-Pugh score, Model for End-stage Liver DiseaseSodium score, C-reactive protein and blood sodium level (p: 0.005, p: 0.009, p<0.001, p: 0.003, p:0.008, p: 0.035 and p < 0.001, respectively). Multiple logisticlineer regres-sion analysis showed that NLR> 3.17 was an independent predictor oftumor size> 5 cm and hyponatremia in patients with hepatocellular carcinoma (p:0.010, p: 0.012, respectively).Conclusions: The value of blood neutrophil to lymphocyte ratio at the time of diagnosis in patients with hepatocellular carcinoma was a good predictor of tumor sizeand grade of disease.

    A Rectovaginal Fistula Treated by Ligation of Intersphincteric Fistula Tract Method

    Rectovaginal fistulas are epithelial-lined communications between the rectum andvagina. A number of surgical options are available for patients with rectovaginal fistulas.In this manuscript, a case having rectovaginal fistula and treated by ligation ofintersphincteric fistula tract method was presented.Twenty-seven years old woman had low settled rectovaginal fistula. We decided tooperate like an anal fistula and we performed ligation of intersphincteric fistula tractmethod. The patient was followed postoperatively for 3 months and she had no complaint.As much as we know, this is a rare case in the literature who was performedligation of intersphincteric fistula tract method.

    Is diabetes mellitus associated with malnutrition in patients in intensive care unit? Diabetes and malnutrition

    Aim: Types of diseases and treatment modalities can also lead to the exacerbation of malnutrition. The aim of this study was to investigate nutritional status of patients with diabetes mellitus in the intensive care unit of a tertiary hospital. Materials and methods: One hundred and ninety-two patients were enrolled and divided into two groups. The study group comprised of 77 patients with type 2 diabetes and the control group comprised of 115 patients without diabetes. The nutritional risk assessment was tested with NRS (Nutritional Risk Screening) 2002, Nutric score, MNA (Mini-Nutritional Assessment) and MUST (Malnutrition Universal Screening Tool). Results: The groups were comparable according to the NRS 2002 (3.37 ± 1.84 vs. 3.93 ± 1.72, p = 0.075), Nutric score (4.61 ± 1.85 vs. 4.56 ± 1.85, p = 0.869), MNA (8.0 ± 3.1 vs. 7.1 ± 3.2, p = 0.068) and MUST score (1.62 ± 1.46 vs. 1.81 ± 1.59, p = 0.456). Conclusion: In this study, the risk of malnutrition is comparable in both groups. This result may suggest that malnutrition is also related to co-morbidities in addition to diabetes.

The cytoskeletal structures are responsible for interconnects between cell nucleus, cytoplasm and also extracellular matrix, whereby itcreate a communication link between cellular molecules and signalization transport.The cytoskeleton is constructed from three kinds of protein filaments as: actin filaments, intermediate filaments, and microtubules. The nature of these structures isbriefly outlined and the literature review that is reporting relationship between cytoskeleton components and common disorders is presented in this paper.

    Effectiveness of Intrauterine Levonorgesterel-Releasing Device in the Treatment of Endometrial Hyperplasia in Obese Patients

    Aim: Endometrial hyperplasia (EH) is a precursor lesion of endometrial adenocarcinoma, the most common gynecological malignancy in women. Endometrial hyperplasias divided in two groups: non-atypical hyperplasia and atypical hyperplasia. The most commonly used treatment approach is progestin therapy for non-atypical hyperplasias. In this study, we aimed to compare the regression outcomes in control biopsies between obese and non obese patients diagnosed with non-atypical endometrial hyperplasia who were treated with an LNG-IUD and followed up in our clinic. Methods: This study conducted was patients were diagnosed with non-atypical endometrial hyperplasia via endometrial biopsy and treated with intrauterine levonorgestrel. Patient data were reviewed retrospectively. Patients were divided into two groups based on BMI: obese and non-obese. İn regression and treatment success were assessed between control endometrial biopsies taken at 6 and 12 months in the obese and non-obese groups. Results: A total of 110 patients were included in the study who were categorized into two groups according to BMI as obese and non-obese. Data of 32 patients in the obese patient group and 78 patients in the non-obese patient group were examined. In obese patients, the regression rate at the 6th month was 62.5%, and the regression rate at the 12th month was 90.6%. In non-obese patients, the regression rate at the 6th month was 96%, and the regression rate at the 12th month was 97.4%. In the obese patient group, both the 6th-month regression rate and the 12th-month regression rate were statistically significantly lower compared to the non-obese patient group. (p < 0.05). Conclusions: Obesity negatively affects the response to progesterone treatment and that regression rates decrease as BMI increases.

    Metamizole Reduces The Expression of OCT4, SOX2, SOX4 in Hematopoietic Stem/Progenitor Cells Directed to Differentiation in In Vitro Hematopoiesis Simulation

    Aim: Metamizole is a non-steroidal anti-inflammatory drug used for its analgesic and antipyretic effects. Metamizole-associated agranulocytosis is a significant adverse effect limiting its use in some countries, but it has also been shown to promote apoptosis in cancer cells with possible mechanisms underlying this adverse effect. The mechanisms underlying these effects have not been sufficiently elucidated. Methods: The cells obtained from stem cell product samples collected from healthy allogeneic stem cell donors were used in the study. MTT assay was used to analyse the proliferative-cytotoxic effects of metamizole on cells. CFU-Mix culture was also performed for cell differentiation to granulocyte and analysis of expression of OCT4, SOX2, SOX4. The expression fold change of transcription factors was determined by RT-qPCR. Results: Metamizole limited the proliferation of cells at increasing concentrations starting at 10 µM. The IC50 values of metamizole were significantly different between the samples. The lowest value for IC50 was 50.96±7.60 µM and the highest value was 165.5±18.21 µM. Granulocyte-macrophage colony counts and OCT4, SOX4 mRNA expressions decreased at metamizole concentrations of 10 and 100 µM. For SOX2, the decrease occurred at a concentration of 100 µM. Conclusion: Our results indicated that metamizole may limit the proliferation and differentiation of hematopoietic stem/progenitor cells via transcription factors and this effect may be responsible for possible agranulocytosis cases. NSAIDs have also been shown to have apoptotic effects on cancer cells. The ability to limit the expression of transcription factors that play important roles in cell development and differentiation may pave the way for the synergistic use of this drug with antineoplastic drugs.

    A Rare Coronary Anomaly: Single Coronary Artery; Two Cases

    We present two cases of a rare coronary anomaly—single coronary artery—identified incidentally during routine coronary angiography. Both patients presented with symptoms of chest pain and exertional angina. In both cases, the anomaly is located in the right sinus of Valsalva (R-1-A) and crosses the heart anterior to the right ventricle. (Figure 3) The first patient experienced significant stenosis in the proximal left anterior descending artery, which was successfully treated with percutaneous stent implantation. The second patient's chest pain was managed effectively with medical therapy alone. These cases highlight that a single coronary artery, often asymptomatic, can exist as a congenital anomaly in patients experiencing myocardial ischemia due to atherosclerotic coronary artery disease. Furthermore, they demonstrate that percutaneous coronary intervention, including stent placement, offers a promising treatment approach. In the literature, coronary artery bypass surgery should particularly be considered as a treatment option for these coronary anomalies. However, it has also been shown that in selected cases, percutaneous intervention or medical therapy can be viable alternatives.

    Investigation of The Effect of Adalimumab on Experimental Brain Injury in Mice

    Aim: We aimed to investigate the neuroprotective role of adalimumab based onthe hypothesis that \"TNF-alpha inhibitor adalimumab may affect inflammationrelated neuronal injury due to its anti-inflammatory effect\".Methods: To investigate the effects of adalimumab, we induced brain injury inmice using a cold trauma model and evaluated the underlying cell survival/ deathmechanisms via cresyl violet and calculated infarct/edema volume with imageanalyze system.Results: Although our data indicated a tendency to decreased infarct and edemavolume, these findings are not significant statistically.Conclusion: To the best of our knowledge, this is the first study evaluating theneuroprotective effect of adalimumab on injured neurons.

    Quartile scores of scientific journals: Meaning, importance and usage

    The Q scores of scientific journals are an analytical tool that determines the ranking of journals based on their own scientific group and impact factor. It shows the rankings of the journal within its own group and it is a useful guide for researchers in the selection of the journal, however it should be used with caution in academic career advancement. Quartile scores may vary according to the scientific categories. The Q scores of a journal briefly show the 25% quantile resulting from quartered the number of journals in the area where the journal is placed. The first quartile has the top 25% of the journals and gets the Q1 score and the last quartile gets Q4 score. And so, the second 25% slice takes Q2 score and the third 25% slice takes Q3 score.

    Correlation Between Suicidal Behavior and Depression, Anxiety, Stress Levels and Sociodemographic Characteristics in University Students

    Aim: Studies conducted on university students have reported high levels of depression, anxiety and stress as well as suicidal thoughts and suicide attempts. This study aimed to determine the correlation between suicidal behavior and depression, anxiety, stress levels and sociodemographic characteristics in university students. Method: Participants who agreed to participate in this online study were asked to fill in the sociodemographic data form and the Depression Anxiety Stress Scale-21 (DASS-21). Among the 530 participants, 517 (97.6%), having read information about the study, agreed to participate, while 13 (2.4%) declined. Results: In this study, 36% of the participating university students had a history of major suicidal ideation, 6.4% currently experienced major suicidal ideation and 14.7% had attempted suicide. Depression, anxiety and stress scores were found to be higher in university students who had attempted suicide or currently experienced major suicidal ideation compared to those who had/did not (p?0.001). With regard to sociodemographic factors; perceived and actual academic performance levels, socioeconomic status, current dieting status, sleep problems, smoking, daily time spent online, chronic diseases, and past or current psychiatric treatment were found to be common risk factors for both current major suicidal ideation and suicide attempt status (p?0.05). Conclusion: Having found high levels of depression, anxiety and stress among university students in this study, we consider that it is necessary to investigate the factors associated with suicidal ideation and suicide attempts, and that comprehensive psychosocial support units and programs should be developed to protect students' mental health and reduce risk of suicide.

Bibliometric studies give researchers a pretty good overall understanding of this area, which is always evolving, and they offer some valuable insights for future investigations. The bibliometric perspective highlights the importance of collaboration. It also highlights the importance of targeted research. These are both important to improve the quality of life of MSK patients.

    Is bilingualism really an advantage? What about individuals with autism?

    The prevalence of autism spectrum disorder (ASD) is rising globally, making the identification of at-risk groups increasingly important. The proportion of bilingual families has also increased in recent years. There are very few studies examining the relationship between ASD and bilingualism. Although bilingualism is considered an advantage in terms of child development, its consequences in the presence of ASD are unclear. Therefore, there is no consensus among professionals in this field on the subject of bilingualism. Being bilingual is a factor that should not be overlooked during interventions for children with ASD. More research is needed to examine the relationship between being bilingual and ASD longitudinally.
