    A Rare Case: Laryngeal Sarcomatoid Carcinoma

    Background: Spindle cell carcinoma (sarcomatoid carcinoma) is a rare carcinoma that accounts for 2-3% of all laryngeal malignancies. Here, we present a case with laryngeal tumor diagnosed as Spindle cell carcinoma. Case Presentation: Fifty-nine-year-old male patient was admitted to our clinic with the complaint of hoarseness and shortness of breath that has been increasing for 10 months. Indirect laryngoscopy performed on the patient, and a polypoid lesion was detected in the anterior of the left vocal cord. The patient was admitted to our clinic for direct laryngoscopy and the lesion was completely excised from the base. The case was diagnosed as sarcomatoid carcinoma in histopathological examination. Conclusion: Sarcomatoid carcinoma is usually diagnosed and treated at an early stage. So, it has a good prognosis. However, it has a poor prognosis in the advanced stage.

    An evaluation of trace elements and oxidative stress in patients with benign paroxysmal positional vertigo

    ObjectivesVertigo and Dizziness are a common complaint among the reasons for applying to the ear nose throat clinic. Benign Paroxysmal Positional Vertigo (BPPV) is the most common cause of perpheric vertigo. Oxidative stress is caused by the formation of hydroxyl radicals, superoxide anions and hydrogen peroxide, which are reactive oxygen derivatives (ROS). The aim of this study is to investigate the relationship between complaints and serum trace element and oxidative stress levels in patients with BPPV. MethodsThis study was conducted with 66 adult patients who presented to the ENT policlinic with the complaint of vertigo and were diagnosed with BPPV between May 2020 and September 2020. Blood samples were taken from patients diagnosed with BPPV to measure serum Zn and Cu levels and oxidative stress levels during an attack. ResultsThe mean ages of the study patients and healthy controls were 45.7 +/- 15.1 and 44.7 +/- 13.2. Female / Male ratio were 28(42.5%)/38(57.5%) and 32(48.5%)/34(51.5%) in study and control group. We found serum Cu levels were lower in the patient group (p <0.05). Serum Total Thiol and Native Thiol values were lower in patients with BPPV. Total Thiol results were statistically significant.(p<0.05) Disulfide values were significantly higher in the disease group. (P <0.05). Thiol Oxidized / Thiol Reduced ratio (2243.6 +/- 6.7/343.8 +/- 125.3) was higher in control group. (p<0.05) ConclusionSerum oxidative stress and trace elements play a role in the pathophysiology of BPPV. We present the cut-off values for Cu and Zn in vertigo patients for the first time in the literature. We think that these cut-off values of the trace elements and thiol/disulfide hemostasis can be used clinically by physicians in the etiology, diagnosis and treatment of vertigo.

    Evaluation of the Presence and Importance of PD-L1 Expression in Head-neck Squamosus Cell Carcinomas and Premalign Lesions

    Background: Programmed Death-Ligand 1 (PD-L1) is a cell membrane protein found on the surface of cancer cells, immune system cells in the tumor microenvironment, and various healthy tissues. Moreover, it plays a key role in suppressing the immune system. Objectives: We aim to explain the presence of PD-L1 in Head-Neck Squamosus Cell Carcinomas (HNSCC) and premalign lesions by immunohistochemical method. Methods: Our retrospective study included 22 patients with HNSCC (15(68.1%) Oral cavity and 7(31.9%) Oropharynx), 20 patients with oral lichen planus, and 14 patients with normal oral cavity mucosa. In the evaluation of PD-L1 antibodies applied immunohistochemically in patients with HNSCC, the percentage of tumor cells showing membranous staining with PD-L1 antibodies was calculated. Results: The mean age of HNSCC patients participating in the study was 52.24 +/- 11.7 years, the mean age of oral lichen planus patients was 34.10 +/- 9.8 years, and the control patients' mean age was 31.42 +/- 10.6 years. The rate of PD-L1 staining of tumor cells of HNSCC patients was significantly higher than the control group (P=0.001). Conclusion: The importance of PD-L1 expression in HNSCC and precancerous lesions of the oral cavity is remarkable, and the values may be related to the pathophysiology of these diseases.

    Investigation of the Proapoptotic Effects of Cetixumab and Selenium Combination on Laryngeal Cancer Cells via TRPM2 Channels

    Background and Objectives: Cetuximab is commonly used as a chemotherapy drug for treating laryngeal cancer. Selenium, known for its antioxidant properties, is also believed to enhance the effectiveness of certain chemotherapeutic agents. This study aimed to evaluate how cetuximab and selenium influence the activation of the Transient Receptor Potential Melastatin 2 (TRPM2) channel. Methods: An observational study was conducted using cultured human laryngeal cancer cells (Hep-2). The cells were divided into seven experimental groups. Cumene hydroperoxide was used to stimulate the cells and activate TRPM2 channels, while antranilic acid was applied in certain groups to inhibit these channels. Measurements included cytosolic calcium levels, apoptosis, intracellular reactive oxygen species (ROS), mitochondrial depolarization, caspase-3 and caspase-9 activities, as well as immunohistochemical (IHC) staining. Statistical analysis was performed using GraphPad Prism 7.04, and data are presented as mean +/- standard deviation. Results: Compared to the cetuximab-only group, cells treated with both cetuximab and selenium showed significantly higher levels of apoptosis, intracellular calcium, ROS, and caspase-3 and caspase-9 activities (P<0.001). Inhibition of TRPM2 channels by antranilic acid significantly reduced apoptosis in these groups (P<0.05 and P<0.001). Conclusion: The pro-apoptotic effect of cetuximab on laryngeal cancer cells appears to be mediated through TRPM2 channels. Selenium enhances this apoptotic response by promoting TRPM2 channel activation and increasing calcium influx. These findings highlight selenium's regulatory role in apoptosis and calcium entry via TRPM2 channels.

    The Role of WNT3A Protein and Gene Variants in Allergic Rhinitis: A Case-Control Study

    Allergic rhinitis (AR) is a prevalent inflammatory disorder of the upper respiratory tract, driven by allergen exposure. Understanding mechanisms and identifying biomarkers for AR could significantly impact diagnosis and treatment. This study aimed to investigate the association between serum Wingless-Type MMTV Integration Site Family, Member 3A (WNT3A) protein levels, WNT3A polymorphisms, and AR. A cohort of 92 AR patients and 86 healthy controls was recruited. Serum WNT3A levels were measured by enzyme-linked immunosorbent assay (ELISA). WNT3A gene polymorphisms (rs752107 and rs3121310) were analyzed using Polymerase Chain Reaction- Restriction Fragment Length Polymorphism (PCR-RFLP) method. The study revealed significantly higher serum WNT3A levels in AR patients compared to controls (p < 0.0001). The impact of WNT3A in the differential diagnosis of AR was determined to be moderate, with an area under the curve (AUC) value of 0.67 (95% Confidence Interval: 0.59-0.75) based on the receiver operating characteristic (ROC) curve analysis. The rs3121310 polymorphism showed a significant association with the GA genotype more prevalent in controls (p < 0.05). However, no significant relationship was observed between rs3121310 genotypes and clinical parameters of the patients. These findings suggest a role for WNT3A in AR pathogenesis, given the elevated serum levels in patients. Larger cohort studies are needed to validate these findings and explore serum WNT3A levels as a biomarker for AR diagnosis and treatment monitoring.

    Upper airway obstruction and nocturnal enuresis in children: Why is it important?

    Objectives: Nocturnal enuresis (NE) is a common urological complaint among children. The most common cause of obstructive airway disease in children is enlarged tonsils and adenoids. Although the relationship between the presence of NE and sleep disorders is unclear, some studies show that enuresis improves after airway obstruction is resolved. We aimed to investigate the relationship between upper airway obstruction and NE in children. Methods: Between September 2020 and June 2021, 66 pediatric patients diagnosed with persistent NE were included in the study. A total of 57 healthy patients were included in the control group. The presence of snoring and apnea, the presence of Attention-Deficit/Hyperactivity Disorder (ADHD)/social adjustment disorder, academic achievement, and family members' history of NE were asked through questionnaires filled out by the families. An upper airway examination was done with a flexible nasopharyngoscope. Results: The mean ages of the study patients and healthy controls were 8.32 +/- 2.1 and 8.18 +/- 2.3 years. The female/male ratio was (25/41) and (33/24), respectively. Of the case group, 62.1% were male, and 78.8% were under nine years old. The frequency of snoring/apnea in children with enuresis was 27.3%, while it was 19.3% in the control group (p=0.299). It was found that more enuresis developed in children with high BMI (p=0.044). Family history was higher in the NE group than in the control group, but it was not statistically significant (p=0.173). Conclusion: Nocturnal enuresis is commonly associated with obstructive sleep apnea. Upper airway obstruction, obesity, and male gender are important risk factors for NE.
