Comparison of Clinical Outcomes on Different Treatment Methods for Lateral Epicondylitis
(2022) Aksoy, Ahmet; Gülcü, Anıl; Aslan, Ahmet
Aim: In our study, the effectiveness of steroid injection therapy, extracorporeal shock wave therapy (ESWT) and splint use in the treatment of lateral epicondylitis, were compared. Methods: A total of 97 patients (28 males, 57 females, mean age: 47 years) with a clinical diagnosis of lateral epicondylitis were included in the study. The patients were divided into three treatment groups. Steroid injection was applied to 30 patients, ESWT treatment to 28 patients, and bandage treatment to 29 patients. Patients were evaluated with Visual analog scale (VAS) and Disabilities of the Arm, Shoulder and Hand (DASH) scores before and after treatment. Results: A decrease in VAS and DASH scores and clinical improvement compared to pre-treatment were observed in all three groups. Although positive results were obtained with all three treatment methods, we found that the most statistically significant treatment method was steroid injection. Better results were obtained in the group that received steroid injection with the peppering technique. (p<0.05).Conclusion: The results of this study showed that local steroid injection, ESWT application and bandage treatment may be beneficial in the short term in the treatment of patients with lateral epicondylitis. However, it was evaluated that the best improvement in scores was obtained with local steroid injection with the peppering technique, whereas the least improvement was obtained with bandage treatment.

Distraction-based systems of early onset scoliosis
(Nova Science Publishers, Inc., 2024) Aksoy, Ahmet
Treatment of early onset scoliosis (EOS) is a challenge for patients and orthopedic surgeons. The aim of treatment is to expand the respiratory capacity of the lung and to create a good lung cavity. The main goal of surgical techniques used in the treatment is to correct the deformity without preventing growth. There are treatment algorithms based on distraction and compression. The distraction-based systems include traditional growing rods, magnetically growing rods, vertical expandable prosthesis titanium ribs (VEPTR), and hybrid distraction-based strategy. If curve progression continues despite nonsurgical management, surgical curve control measures are instituted. The best surgical option is dictated by the overall clinical condition of the individual patient. © 2024 Nova Science Publishers, Inc. All rights reserved.

Our clinical and radiological results in the treatment of bone transport with ilizarov external fixator in patients with segmental tibial defects. Is depression associated with poor outcomes?
(Wolters Kluwer Medknow Publications, 2023) Aksoy, Ahmet; Atay, Tolga; Aslan, Ahmet
Objective: The aim of this study was to evaluate the clinical and radiological results of patients who underwent bone transport with Ilizarov external fixator in the treatment of tibial defects due to traumatic causes, and to investigate the factors that have a positive or negative effect on the results. Patients and Methods: From a total of 30 patients with tibial segment loss due to traumatic causes treated with bone transport with an Ilizarov external fixator between 2013 and 2020 in the Department of Orthopedics and Traumatology of the Faculty, 25 with adequate examinations and follow up were included in this study and were evaluated retrospectively. The patients were grouped according to the etiology of bone loss, demographic characteristics, Paley's pseudoarthrosis classification and Solomin's long bone defect classification. The results were evaluated according to the Association for the Study and Application of the Method of Ilizarov (ASAMI) outcome criteria. Results: The 25 patients in the study comprised 20 males and 5 females with an average age of 44.80 +/- 18.51 years. According to the Paley pseudoarthrosis classification, 16 (64%) patients were Type B1, 5 (20%) were Type B2, and 4 (16%) were Type B3. According to the Solomin long bone defect classification, 4 (16%) patients were Type B2, 14 (56%) were Type C1, and 7 (28%) were Type C2. In some patients, due to the fixation problem, early fixation was achieved with minimally invasive plate osteosynthesis or intramedullar nailing by removing the Ilizarov fixator after the necessary length and alignment was achieved for the purpose of early rehabilitation. Early fixation was achieved before consolidation was completed in 16 of 25 patients. Fixator duration was 360 +/- 130.77 days in the group without additional fixation and external fixation index (EFI) was 61.63 +/- 29.81 days/cm, while the fixator duration was 273.33 +/- 116.82 days in the early fixation group, and EFI was 57.22 +/- 29.05 day/cm. Complications were evaluated according to Paley's criteria. A total of 39 complications were recorded, the most common being pin tract infection. The ASAMI bone results were found to be excellent in 7 patients, good in 9, moderate in 4, and poor in 5. The functional results were excellent in 5 patients, good in 11, moderate in 4, and poor in 5. In addition, 5 (20%) of 25 patients whose results were evaluated had depressive illness. Moreover, the results of these patients were found to be poor according to the ASAMI criteria. Conclusion: The Ilizarov method is a very safe and successful option in the treatment of open fractures with tibial segment loss resulting from high-energy traumas. Nevertheless, when making the decision for treatment with this method, the patient's current depressive disorder should be taken into consideration in addition to the clinical problems of the patients.

Radiologically Guided Versus Blinded Intra-articular Injection in Patients With Hip Osteoarthritis: A Retrospective Comparative Study
(2022) Aksoy, Ahmet; Gülcü, Anıl; Tuna, Mehmet Mert; Aslan, Ahmet
OBjeCTIVeS: The aim of this study was to present the clinical results of patients with Kellgren-Lawrence (KL) stage 2-4 hip osteoarthritis who were administered intra-articular corticosteroid (CS) or hyaluronic acid (HA), with or without fluoroscopy.MeTHOdS: This retrospective comparative study was conducted in the clinics where the authors worked between 2010 and 2018. Patients with stage 2-4 hip osteoarthritis according to KL criteria were included in the study. Age, body mass index, American Society of Anesthesi-ologists stages, and Western Ontario and McMaster Universities Arthritis Index (WOMAC) scores (3rd, 6th, and 12th months) were recorded. Two groups were created as patients who underwent injection with or without fluoroscopy guidance. In group 1, CS (triamnisolone) was administered, and in group 2, sodium hyaluronate 88 mg/4 mL was administered. Obtained parameters were compared.ReSulT S: The WOMAC scores at 3 months of both the CS and HA groups were statistically significantly better than before the application, with the improvement in the CS group found to be significantly better than in the HA group (P = .047). At 6 months, the mean WOMAC scores of the CS and HA groups were better than prior to the application, and there was a statistically significant difference (P< .001). No significant differ-ence was found in either the CS or HA group in the comparison of 12-month WOMAC scores with the baseline scores (P = .744 and P = .054).COnCluSIOn: In symptomatic hip OA patients, intra-articular administration of CS and HA was seen to be effective at 3 and 6 months after administration. However, the effectiveness was determined to have disappeared within 1 year. Furthermore, in hip OA intra-articular drug applications, it was determined that the blinded technique without radiological guidance performed in the outpatient clinic is as effective and safe as the radiologically guided technique administered in the operating room
